Geriatric Psycho-Social Assessment
A geriatric psycho-social assessment is the crucial first step for senior rehabilitation needs and gaining clarity on the comprehensive care required by aging parents. This all-inclusive assessment evaluates your loved one’s emotional, mental, medical, social, physical, and living arrangement needs. Following the assessment, we provide a detailed written report containing all the gathered information to guide senior rehabilitation efforts and facilitate the provision of appropriate care.
Geriatric Care Management
Through our ongoing geriatric care management, we help you manage your aging loved one’s essential daily needs. This may include such support services as:
- Nursing assessment/medication management
- Coordinating medical appointments
